2025 ACR Guidelines for Systemic Lupus Erythematosus: A New Era of Earlier, Targeted, and Steroid-Sparing Care

Systemic lupus erythematosus (SLE) remains one of the most complex autoimmune diseases encountered by rheumatology professionals, characterized by diverse clinical manifestations, unpredictable disease activity, and the potential for irreversible organ damage. In 2025, the American College of Rheumatology (ACR) released updated guidelines for the treatment of SLE, emphasizing a significant shift in management: earlier use of disease-modifying therapies, reduced glucocorticoid use, and individualized treatment strategies to prevent long-term damage. This is the first ACR guideline specifically for the treatment of non-renal SLE, filling a longstanding gap in U.S. rheumatology practice.

For rheumatologists and referring providers, understanding these updates is essential as the therapeutic landscape of lupus continues to rapidly evolve.

  1. Hydroxychloroquine Remains the Foundation of SLE Management

The ACR continues to recommend hydroxychloroquine (HCQ) for nearly all patients with SLE unless a contraindication exists. Decades of evidence support its role in reducing disease flares, improving long-term survival, lowering the risk of thrombosis, and reducing organ damage.

The guideline emphasizes maintaining a weight-based dose of ≤5 mg/kg/day of actual body weight to minimize the risk of retinal toxicity while preserving the significant benefits of long-term therapy.

  1. Moving Away from Chronic Glucocorticoid Dependence

One of the most important themes of the 2025 guideline is a stronger commitment to minimize glucocorticoid exposure.

While corticosteroids remain valuable for controlling moderate to severe disease flares, prolonged exposure contributes significantly to irreversible damage, including osteoporosis, cardiovascular disease, infection risk, cataracts, diabetes, and avascular necrosis.

The ACR recommends:

  • Using the lowest effective dose for the shortest possible duration
  • Avoiding long-term prednisone when alternative steroid-sparing therapies are available
  • Initiating or escalating immunomodulatory therapy early to facilitate steroid tapering

The guideline identifies prednisone ≤5 mg/day as a preferred maintenance goal whenever feasible.

  1. Earlier Use of Conventional and Advanced Therapies

The treatment paradigm for SLE has expanded significantly beyond traditional immunosuppressive agents.

For patients with persistent disease activity despite hydroxychloroquine, the ACR supports early consideration of additional therapies, including:

Conventional immunosuppressive agents

  • Methotrexate
  • Azathioprine
  • Mycophenolate mofetil

These remain important options, particularly for patients with musculoskeletal, cutaneous, or organ-threatening manifestations.

Biologic and targeted therapies

  • Belimumab, a B-cell activating factor (BAFF) inhibitor, can improve disease activity and reduce flares.
  • Anifrolumab, a type I interferon receptor antagonist, has demonstrated efficacy particularly in patients with moderate-to-severe non-renal SLE.
  • Rituximab, an anti-CD20 B-cell depleting therapy, remains an important treatment option for patients with severe, refractory manifestations of SLE, particularly in cases of severe hematologic involvement, refractory lupus nephritis, or other organ-threatening disease.
  • Cyclophosphamide, a potent alkylating immunosuppressive agent, continues to have a role in severe, life-threatening SLE, including severe lupus nephritis, neuropsychiatric lupus, and severe systemic disease requiring rapid and aggressive immunosuppression.

The choice of agent depends on the nature, location, and severity of organ involvement, as well as age, childbearing potential, safety profile, comorbidities, and patient preference.

Overall, the updated guideline reflects a shift away from the historical approach of waiting years before adding advanced therapies. Earlier intervention may allow better disease control and reduce dependence on corticosteroids.

  1. Treating Beyond Symptoms: Preventing Long-Term Damage

The goal of lupus treatment extends beyond controlling flares. The ACR emphasizes achieving sustained low disease activity or remission while preventing irreversible damage through minimizing glucocorticoid exposure, monitoring for medication toxicity, addressing cardiovascular disease and bone health, maintaining appropriate vaccinations, and managing associated comorbid conditions.

Looking Ahead: The Evolution of Lupus Care

The 2025 ACR SLE treatment guideline represents an important transition in lupus management. With an expanding arsenal of targeted therapies, clinicians are increasingly able to control disease earlier and more effectively while minimizing the complications associated with chronic corticosteroid use.

For rheumatology providers, the key message is clear: early disease control, steroid-sparing strategies, and individualized use of both conventional and advanced therapies to guide lupus management.

As new therapies continue to emerge, the future of SLE care is moving toward a more precise, personalized approach—aiming not only to reduce disease activity today, but also to preserve organ function and quality of life for decades to come.
